    Motions:
    I recommend award of the consultant contract with Forever Young Academy of Dance, Inc. for $21,800.00, effective October 1, 2006 through June 30, 2007, and authorize the Superintendent and Board Chairman to sign all needed documents. - PASS
    Minutes: CC1 The Consultant shall perform the following services: Forever Young Academy of Dance is a dance, gymnastics and cheerleading program for students that is designed to encourage teamwork; physical fitness, improvement of motor skills and improvement of self-confidence. Service will be provided during the Middle School After School Program.

    Minutes: CC5 The Consultant shall perform the following services: Consultant will instruct students in the fundamentals of performing a variety of magic tricks using everyday objects such as: coins, cards, dollar bills, paper clips and pencils. The emphasis will be on having fun and developing an ability to entertain family and friends. Consultant also performs a magic show with student participation.

Upon completion of each workshop students will have an executable version of their game, which will be theirs to keep.

    Minutes: CC11 The Consultant shall perform the following services: Consultant will provide a variety of activities including: Brain Game, a program designed to help students learn curriculum based information; Star Studio, a program designed to help students create their own compact discs through karaoke sound tracts; DJ Dance, with special effects; & a Design Your Own Photo T-Shirt activity.

  14. CC12 Consultant Contract for EBS Healthcare
    Motions:
    I recommend award of the consultant contract with EBS Healthcare. for $80,000.00, through June 5, 2007, and authorize the Superintendent and Board Chairman to sign all needed documents. - PASS
    Minutes: CC12 The Consultant shall perform the following services: Provide one bilingual (Spanish) speaking speech/language pathologist to fill a vacancy on the West Area's Child Find Diagnostic Team. This SLP is responsible for assessing, report writing, providing interpretations, and participating in meetings as required for children referred to Child Find. The consultant will furnish the District a certificate of professional liability insurance and a valid Florida speech/language pathology license for the bilingual SLP. The SLP will bill Medicaid through the District for eligible students as appropriate.
